New Student RegistrationThe following List is a checklist of documents and forms which will need to be presented at the time of your child's registration:
1 ________ Birth Certificate 2 ________ Immunization Documents 3 ________ Physical examination (grades K-1 only) 4 ________ Dental examination (grades K-1 only) 5 ________ Proof of Address: Parent/Guardian must present 2 different current bills linked to the home to verify residence (PG&E, Water or Garbage service and Current Mortgage statement, County Property Tax bill or Final Escrow Papers.) Rental/ Lease Agreements may be used along with one additional current utility bill linked to the residence. 6 ________ Parent/Guardian Identifcation |
